In May 2019 contractors moving a roughly three-tonne coin blanking press by forklift let it topple, narrowly missing a worker. CDPP had filed three charges alleging no safe system of work and an inadequate SWMS for the contractor; the undertaking was accepted instead of prosecution.
Outcome
Comcare accepted an enforceable undertaking (23 June 2026). Terms include: WHS undertaking ~ $1.1M: industry mobile plant/contractor guidance and training, gallery safety display, safety culture program, AIHS forklift-pedestrian program; prosecution (3 charges) discontinued. An undertaking is not a finding or admission of a contravention unless the regulator says the party admitted it.
