In December 2023 a worker's arm was drawn into a glass crushing plant at the Seaford Heights material recovery facility after a sleeve or glove caught between roller and belt. The company had been charged with a Category 2 offence before the EU was accepted.
Outcome
SafeWork SA accepted an enforceable undertaking (2 June 2026). Terms include: EU min $347,880: external machine-safety reviews, guarding/exclusion upgrades, LOTO training, ISO/AS certifications, industry forum and video, community education, charitable donation. An undertaking is not a finding or admission of a contravention unless the regulator says the party admitted it.
